Step by Step Process

1. Register for a Colonial Account

2. Once registered, log in and click on Claims Center

3. Click File an Online Claim, to file many types of claims, such as: 

• Wellness
• Doctor’s Office Visits
• Accident
• Sickness
• Short Term Disability (Important Note Below)

• etc..

You may also download claim forms and find answers to many of your claims related questions.

IMPORTANT NOTE RE: DISABILITY CLAIMS

After using the full 6-month benefit the disability plan will be exhausted and terminated.  You will need to call us to re-enroll in the coverage when you return to work.
